HEALTH RECORDS

De-wormer Medications

Given at 2, 4, 6, and 8 weeks of age: See Vaccination Record as puppies will have at least 2 more until 12 weeks of age ( your puppies vaccination record is attached).

Vaccines

One age appropriate (6.5 weeks) 5-way vaccine given June 14,2024 Mfr: VUN/PCN 196 vaccine includes Canine Distemper - Adenovirus Type 2 – Parainfluenza - Parvovirus Vaccine (modified live virus). Lot#: E100548A

Important Vaccine information:

Your puppy has received age-appropriate vaccines in its first 8 weeks. If your puppy is less than 12 weeks it has not received it’s 12 week vaccinations (but has received the 5-way vaccinations as listed above), These are considered its “core vaccines.” Please confirm with your vet to see if it will need to have these same vaccines given at 12 and 16 weeks before they will be considered immune to the diseases or safe. Usually, Rabies is given at 13-16 weeks of age.

HEALTH WARRANTIES AND GUARANTEES

General Health Warranty

At the time of purchase, breeder guarantees this puppy is in good health, is up to date on core vaccines and de-worming medications and to the best of our knowledge, free from any diseases and/or birth defects. If this puppy is ever diagnosed with a serious disease or mild to moderate recurring health issue(s), breeder requests to be notified. Keeping track of our puppy’s health into adulthood is important to us.

Puppy Return Policy

Buyer can return the puppy to the breeder 12 days after receipt; but due to costs incurred by the breeder, there will be no refund. The puppy must be seen by a veterinarian and receive a clean bill of health before a return authorization code will be issued. Buyer must pay all return costs (i.e. shipping).

Buyer must have the puppy examined by a Veterinarian within 12 days of receiving this puppy.

If this puppy is diagnosed with an illness, disease, or birth defect by a veterinarian within 12 days of purchase, puppy may be returned for a full refund (excluding the $____ deposit minus

$_____(usually shipping costs). All records of the diagnosis must be provided to breeder within 3 days from the date the puppy was diagnosed.

General Health Warranty Limitations

After the post adoption vet check-up and the 12-day return policy period has passed, the health guarantee is in force. This health guarantee does NOT cover non-genetic or genetic diseases or general health issues as your puppy’s parents are genetically clear according to the EMBARK test we have attached. A one-time, (from receipt up until 4 months of age), $400 vet bill reimbursement will be given to the original contracted owner of said puppy if the puppy has any non-genetic/unspecified illness that does not fall into the following category(s): Such as but not limited to: accidental causes (such as a car accident), viral illnesses, internal or external parasites, bacterial infections, food or skin allergies, broken bones/tendons, diabetes, hernias, dental issues or improper bites, heart murmurs, non-congenial heart disease, gastrointestinal or digestive issues, protozoal diseases, such as giardia or coccidiosis, urinary tract infections or cancer. It does not cover illnesses due to the indigestion of foreign objects, food, chemicals, stress related issues, behavioral issues, physical injuries, or illnesses which may have occurred, developed or contracted after possession of this puppy was transferred to the buyer.

Genetic Health Guarantee

A genetic health guarantee is offered to the buyer for 16 months from the birth date of this puppy (August 28, 2025).. This genetic health guarantee includes hereditary, congenital or genetic defects that would cause this puppy/dog to lose quality of life before the age of 5 or shorten their life span. All medical records MUST be provided to the Breeder from a Veterinarian genetic specialist concerning an inherited genetic disorder, defect or disease. If a genetic diagnosis is confirmed and documented proof has been provided, the Buyer will have a refund equal to one-third of the puppy’s original purchase price (not including shipping or destination fees).. The Breeder does NOT require this dog to be returned and a replacement puppy will not be ordered. The Breeder will assist the Buyer in the rehoming process by using reasonable means of support to reach out to other potential puppy parents for placement - if the Buyer desires, and if the owner should decide to rehome the puppy/dog after receiving a serious genetic diagnosis, the puppy/dog must be rehomed from the Buyers location. The Breeder is not financially or legally liable or responsible for any additional fees or expenses acquired by the Buyer including travel expenses or medical fees to diagnose, care for, or to return the puppy/dog (an and all returns require a Return Authorization code),

Rehoming

Under no circumstances should this puppy/dog ever be relinquished to a pound, shelter or rescue without first consulting the breeder. The genetic health guarantee is not transferable to a second owner. No other rehoming stipulations apply.

Photo Rights

We love seeing pictures/videos of our dogs after they are in their forever homes! Breeder retains the right to use any photos/videos of this puppy/dog for any use including, but not limited to, advertisement etc.

Keeping Your Puppy Safe

Please understand this puppy is not fully immune to contagious diseases yet. Not until they’ve completed all their core vaccine boosters. Currently, it is not safe for them to visit high canine traffic areas in public areas such as dog parks, camping grounds, groomers, lakes, beaches, or even dog friendly stores like Home Depot, Lowes, etc. At the vet’s office, when your puppy is not on the exam table, they should be in a carrier or in your arms. Do NOT let them wander and sniff the clinic floors and definitely do not let them on the grass outside the clinic. The Parvo virus can survive on surfaces and in the soil for over 6 months. People take their sick dogs to the vets office, so this is where you need to be MOST vigilant. Keeping your puppy away from areas in public is most important but do NOT hide but just limit your puppy’s exposure to everyone and everything until they’re done with vaccines at 16 weeks old. The first 4 months of a puppy’s life is also the most important time to socialize them. Introducing them to as many new people, new dogs, and new experiences is the goal. It’s important to take them out of your family bubble to meet new friends and new dogs! Introducing them to dogs you know to be safe and healthy. The Breeder also highly recommends signing your puppy up for Puppy Training Classes at least 1 week after their 12-week appointment or second set of vaccine boosters. Same advice goes for taking walks or public outings in low canine traffic areas like your neighborhood. ALWAYS check the walking surface for excess heat or cold, taking water along. NEVER EVER leave your pup in a hot car!

Legalities

The Breeder neither makes nor implies any warranties or guarantees, express or implied, other than those written in this agreement including without limitation, the warranties of merchantability and fitness. This document constitutes the entire agreement between the Breeder and the Buyer with respect to this sale. Buyer’s signature below and copy of valid, current driver’s license (please attach; you may erase or cover up personal information such as the last 4 digits of any control number (except driver’s license number and weight. However the following elements must be included from your driver’s license: Full Name, Address, License number, signature and expiration date) indicates they have read, understand, and agree to all the conditions of this Sales Agreement and Contract of Sale. Any disputes, mediation or legal proceedings would be held in Valusia County, FL., USA. Both Breeder and Buyer agree to uphold all covenants of this agreement.
